On my 1099-R, box 15 asks for the State Number but my form I received from the bank does not have one listed. How do I continue?

If your form does not have a State ID number in Box 15, you can use the PAYER'S TIN number.  This is located to the left of your SSN on Form 1099-R.

What if is a NJ Pension? Do I use the FIN. Turbo tax entered a W in the box in the program

Yes, the FIN, FEIN, TIN all identify the federal tax id number and can be used in place of the state id - if you need to enter the information.
