On my 1099-R form I see that federal and state tax was withheld. However on my 1040, the taxable amount was total amount with no withholding subtracted.

The withholding isn't subtracted before it goes on line 4a of your 1040. Your withholding is reported on line 16.
Your withholding then gets subtracted from the tax due, on line 11.
