Our 401k/Roth IRA contributions are on our W2. Do I need to add them separately on the app when it asks did we make contributions to these types of accounts?

No!   Your retirement contribution is added to your tax return when you enter your W-2.  Do not add it anywhere else.

And your Roth account in the employer plan is not a Roth IRA.  It is a designated Roth account in the 401(k) plan (Roth 401(k)).
