Box 12 of my W-2 accounts for SIMPLE IRA contributions from my work and has been accounted for when I entered my W-2 data. Later I am asked to advise if I have contributed any monies to Traditional and/or Roth IRAs. Should I check the "Traditional IRA" box and re-enter the same amount I entered in Box 12?

No ... a SIMPLE IRA   is not the same thing as a TRADITIONAL IRA.  Do NOT enter the contribution again since it was handled on the W-2 screen.
