Referring to my 1099R the amount of federal tax withheld is $4,253.88 but Why is it not reflected in the REFUND box as no taxes are required?

The amount of  federal taxes on the 1099R should be refunded.

The 1099-R box 4 is tax that was withheld just like W-2 withholding.

All W-2 and 1099 withholding is added together and goes on line 16 of the 1040 form and will be subtracted from your total tax liability (line 11).   If all of your tax withheld and other credits are more than the tax liability, you get that back as a refund.   If less than the tax liability, you will owe additional tax.
