Removing the "X" for qualified retirement plan on the IRA Contribution worsheet

I have a part-time job offering a SIMPLE IRA. I don't participate and no contributions have been made in my name by my employer. The box for retirement plan is not checked on my W-2. However, an "X" appears in the box for a qualified retirement plan on the IRA Contributions worksheet.  As a result, TT says my IRA contributions made based on my sole proprietor income are not deductible.  Reading the IRS publication indicates that they are deductible.  How/where do I uncheck the box regarding eligibility for a qualified retirement plan such that the contributions become deductible?

One possibility is that there is a code in box 12 on your W-2 form that would indicate that there are contributions to a retirement plan, specifically code D - H or AA or BB. If so, you may need to leave that entry off if it does not affect your income tax and you are sure you are able to make retirement plan contributions this year.
