Rollover - After Tax 401k to Roth IRA - form 5498

I rolled over my 401K after tax contribution to a Roth IRA ---- rec'd a form 5498.

 

Do I need to file any of the info in my taxes for this transaction ---- Rollover amount and FMV of the account ?

You must enter the Form 1099-R that reports the distribution from the 401(k) which was rolled over to the Roth IRA, then answer the questions that follow to indicate that the money was rolled over to a Roth IRA.
