I believe a qualified Roth distribution to a disabled, under 59 1/2 yr old is not subject to income tax nor penalty tax.

Where do I provide this info on TurboTax?  It's in IRC sec. 408A(d).

To qualify for a non-taxable distribution from a Roth IRA before age 59-1/2 for disability, the account must also have been open for five years.  You may also have a Code T in Box 7 on your 1099-R to indicate a Roth Distribution with Exception.

Click the link for more info on Roth IRA Distributions

To enter your Roth Distribution in TurboTax, go to the Wages & Income section of your Federal return and choose IRA, 401(k), Pension Plan Withdrawals (1099-R).

Enter your 1099-R info exactly as shown and pay close attention to the follow-up questions in the section.
