Yes, you should receive a Form 1099-R if you rolled over IRA funds into a 401(K) account during 2018.

Form 1099-R  must be sent out to you by January 31st, so you should receive it in the next couple of weeks. In addition, most financial institutions have the form available on their website so you may find it there.
