Should the contribution amount on line 2 of my husband's 5498-SA be entered in "any contributions you personally made" in the HSA contribution section?

If your husband's HSA account is through his employer and he receives contributions through his employer on his W2, then NO you should not enter the amount in Line 2 as personally made contributions.

 

If your husband has a plan through his employer, then you will see a Box 12, Code W entry on his W2. 

 

"Contributions you personally made" are contributions that you made by writing a check or transferring money to the HSA. This does not include money that was paid through his employer and appears on the W2 (though that maybe your money too).
