Question

The 1099r I received has the code 4 listed in box 7 but I'm still alive. How should I fix this?

A code of 4 does not mean that you died, It means that the original owner of the account died and that is the reason for the distribution. Nothing needs to be fixed, just enter the 1099-R exactly as it is.
