There was no roll over, this the amount we get each month divided by 12. this is our retirement money to live on not a roll over.

What is your question? If you took a distribution from your retirement plan, just enter your 1099-R exactly as it appears. Do not answer any questions about rollovers in the affirmative. You did something else with the money. It doesn't matter what, as long as you do not say it's a rollover.
