Form 5329 is not needed to report contributions to an HSA.  Did you have distributions from your HSA?  If you did, and you didn't indicate that the funds were used to pay for qualified medical expenses, then they would be taxable and you would need Form 5329. It is also used to report additional taxes on IRAs, other qualified retirement plans, modified endowment contracts, Coverdell ESAs, QTPs, Archer MSAs, in addition to HSAs.  Is it possible you need Form,5329 for one of these?
