WHAT SHALL I DO IF MISSING BOX 15-STATE NUMBER ON 1099R LAST YEAR I DIDN'T USE A NUMBER FOR UTAH BUT THIS YEAR THE ONLINE VERSION WILL NOT ACCEPT IT WITHOUT THE NUMBER.

    If your 1099-R is missing the state number in Box 15 and TurboTax won't accept it, here's what you can do:

    1. Contact the issuer: Reach out to the employer, payer, or issuing agency to request the correct state payer's identification number for Utah.
    2. If you can't get the information from the issuer, you can contact the IRS for help. If you don't receive the missing information by the end of February (already past), call the IRS at the number on this page. The IRS can contact the payer to request the missing form
