What to do with a 1099-R received for previous tax year?

I received a 1099-R in January of 2017 but the date on the form is 2015.  The withdrawal occurred in February of 2015 so I assume that's why the date is 2015.  I filed my 2015 taxes a year ago.  Why did they just send me this form?  What do I do with it now?

The information could have been wrong the first time they sent it, or it could be due to them completely forgetting. The best way to know would be to give the sender a call to ask them about it, as they are the only one who knows. You can file an amended return if you need to change or add information for a return that's been submitted already.

To File an Amended Return:

1. Wait for your return to be accepted or rejected by the IRS

2. Log into your account, and at your tax timeline select your "2015 Tax Year"

3. Select to "Amend (Change) 2015 return" and follow the directions

4. Scroll down to "Investment Income", select "show more"

5. Select "Stocks, Mutual Funds, Bonds, Other"

6. Continue through the interview questions until you arrive at the screen that says "Let Us Enter Your 1099-B"
