When finalizing my state (PA) return I am presented with a message to be sure to attach a copy of my 1099-R as I held state taxes on my 401(k) withdrawal.

When finalizing my state (PA) return I am presented with a message to be sure to attach a copy of my 1099-R as I held state taxes on my 401(k) withdrawal.  How do I do this?  I do not see it listed in the Review Forms List (if I go to the delete a form screen) so I assume it is not included when filing state.  It is in the Federal Return list.

You will need to send the state a paper copy of your 1099-R.  This is not something you will print from TurboTax, it is the copy you received from the plan administrator.

Why doesn't TT fix its software so that a user can attach the 1099s to its TT electronically filed return? It seems confusing and inefficient to have to mail the 1099s separately.

While TurboTax does not support attaching all supporting tax forms yet, it will alert you of the necessity to mail in any supporting forms and provide instructions to do so if it is required.
