No, you would not subtract the employer contributions from the total distribution to determine your contributions.  These numbers may not be related at all.

If all of your contributions to the Health Savings Account (HSA) were made through payroll deductions, then the amount shown on your W-2 in box 12 with code W will include contributions that you made and contributions made by your employer.  They are added together.  Any amount reported on your W-2 in this manner does not need to be broken down further between your contribution and your employer's.

When you go through the Deductions and Credits > Medical > 1099-SA, HSA, MSA section of your return and you see a question about any contributions that you made to the HSA, it is referring to any separate contributions that you made outside of any payroll deductions.

Entering information from a Form 1099-SA reporting that you took a distribution from your HSA is a separate thing.  The amounts reported on the 1099-SA are not necessarily related to the amounts that were contributed to the account for the year.  Instead, it is reporting money that you took out of the account to spend on medical expenses.
