Which tax form did you receive for the death benefits?  Form 1099-R?

If you did receive a Form 1099-R, this would be considered a pension or retirement distribution.  If that's the case then you report it using the steps in the link below.

If you have not yet received a tax reporting document, check with the company that made the payment of the survivor lump sum benefits.
