Where do i put insurance deducted from my 1099-r Form from my state retirement?

It was not listed an amount in line 5 but shows total insurance deducted by the account number below. so where do i put this, in line 5?

You can enter it in Box 5.

If this is health insurance and you are itemizing your deductions, you can also enter it under Medical Expenses as insurance.
