Where do I report my IRA RMD income?

It is on form 5498, not on form 1099-R

A Form 5498 is an informational form only.  It is not entered on a tax return.

 

If you had a distribution from the IRA in 2021 then you should have received a Form 1099-R from the plan administrator.  Only the 1099-R can be entered on your 2021 tax return.

If you had a distribution in 2021 and have not received the form 1099-R then you need to contact the plan administrator.
