Where is the Payer information listed on the 1099-SA to fill in the blanks on Turbo Tax?

The first blanks when you're entering your HSA information on Turbo Tax ask for the Payer name and address, but it doesn't show that on my 1099-SA.

Please review the attached image of a 1099-SA. I have highlighted where you would find the HSA information and TIN.
