Question

Why did I receive a 1099-R from my employee retirement plan? I've never received one before and didn't take out any money from those accounts so am very confused.

I've searched around and read through several articles, but it's not making sense to me. I didn't rollover any accounts or take anything out of them. Can someone break this down?

What is the code in box 7 of the Form 1099-R?

7 is the code which in checking the form means "normal distribution."

Then your plan administrator has posted the Form 1099-R because they have information that you were sent a distribution from your account.  If there is an amount in box 1 of the 1099-R and you never received those funds then you need to contact the plan administrator.  The name and phone number should be shown as the Payer on the Form 1099-R.
