This will happen if you mistakenly put a zero in box 2a or leave it blank on TurboTax's 1099-R form. A code 1, 2 or 7 Form 1099-R with the IRA/SEP/SIMPLE box marked must always have bx 2a equal to box 1 and have box 2b Taxable amount not determined marked.
