In 2021 I had total of 3 -W-2 forms for a total approx $16500.00. I also received one 1099 NEC for some work as a door dash driver. Do I just add the NEC amount to adjusted gross income line on the IRS short form? Or must I fill out another sep form?

You need to include the income from your 1099 as self-employment income.

Follow these steps to go to the Schedule C section of your return:

On the top right corner of the TurboTax online screen, click on Search (or for CD/downloaded TurboTax locate the search box in the upper right corner).

    1. Type in “schedule c” (or for CD/downloaded TurboTax, click Find).
    2. Click on “Jump to schedule c”.
    3. Click on the blue “Jump to schedule c” link

Thank you. So In addition to the 1040, that has the W-2 info a schedule C must be filled out and attached when sending?

Yes, as a contractor you are considered Self-employed by the IRS.  Follow the instructions given by @ColeenD3 and also review What self-employed expenses can I deduct?
