If nothing has changed, you should request a Form 1099-R instead of Form 1099-MISC.

The IRS will have a copy of the form that you received and if you enter the information as if it were reported on a Form 1099-R that could cause some red flags. There is much more information reported on Form 1099-R that is needed in order to be sure that your income is taxed properly. You should not create that information yourself based solely on Form 1099-MISC.

I would not recommend that you enter the information as if it had been reported on Form 1099-R.
