How do you fill out business information for 1099-nec if it isn’t your business and your being paid by someone else?

It depends. The business is paying you as an independent contractor, which means you are not an employee, rather you are self employed.

 

The business name is your name and the business address is your address unless you have a separate business location or created your own business name legally.
