What info should I fill out under the Business name 1099 if I am a sub-contractor?

I am a physical therapist assistant for home health care. I am a sub contractor for a company. I have a 1099-NEC to fill out. I entered all the information under the 1099-NEC section in Turbo Tax but under the Business Income and Expenses section, it asks me for "Business Profile" It says: " We used your name and profile for your business.If you need to change it, you can do it here." And then the 2 options are: 1) I USE MY OWN NAME FOR MY BUSINESS or 2) MY BUSINESS NAME HAS A DIFFERENT NAME THAN MY OWN. I am confused because non of those apply to me since I work for that business. When I hit next, it then asks me for "Any missing business info." So my question is, should I just put the name of the company along with their address for both sections? THANKS

    Since you don't have a business registered and you're a subcontractor, just leave it with the information that TurboTax entered, which is your name and address.  Since you are a subcontractor you will report this income as if you were self-employed.
