How do I claim money I received for equipment and my studies from "Federal/Provincial Grant for Post-Secondary Students with Permanent Disabilities Schedule 4" ?

I received a full student loan from the NSLC and Provincial Student Aid, and this money was granted to me to assist with my learning disabilities. And students with permanent disabilities are eligible for up to $4000 yearly.
