1) I have 4 1099-R's - turbotax lets me enter the data for all of them, it calculates my tax correctly for both Federal and NY State tax.
2) NY State requires that a copy of all 1099Rs (as well as W2s) be included in the return.
3) When I create a record of my return (I have not submitted them), only 3 1099R's are printed
the first page has 1099R #1 and #2
the second page has 1099R #3 and 'instructions'
nowhere is 1099R #4 shown.
4 even just creating a pdf or printing NY State form IT-1099-R, only shows three of the 4 1099R content but consistently the instructions in the picture above show beneath the 3rd one.
Please let me know if something specifically is unclear.
If you entered all of your Forms 1099-R and your income and taxes are calculated properly (including all Forms 1099-R) than TurboTax has included them all in your income. You are e-filing your NYS tax return, correct? They will be transmitted to New York State correctly. You do not need to mail them.
Have you tried previewing your return to see if all Forms 1099-R have been properly input and are included in your income?

Select Tax Tools from the menu (if you don't see this, select the menu icon in the upper-left corner).
With the Tax Tools menu open, you can then:
Preview your entire return: Select Print Center and then Print, save or preview this year's return (you may be asked to register or pay first).
View only your 1040 form: Select Tools. Next, select View Tax Summary in the pop-up, then Preview my 1040 in the left menu.
You can preview your return in the TurboTax Desktop software by following these steps:
With your return open in TurboTax, select Preview from the File menu at the top. (If you don't see "Preview," open your return, not just the program).
In the pop-up window, choose the set of forms you wish to see (all official forms, specific forms, etc.) and select Preview.
Scroll down to view all the forms if you choose to preview multiple forms.

No, that 1099-R is omitted because it is not part of the NY state return. NY state has received a copy of it separately and will not be expecting it to be included. You're all good to go.
