Why is the state of MI making me complete a form 5049? I am married filing separately. We lived together. I'm stuck can anyone help?

Do I  just fill out the form?

Yes. Per the State of Michigan, this form must be filled out if you lived together and your filing status is married filing separate. This form is intended to assist you in correctly calculating total household resources for the Homestead Property Tax Credit or the Home Heating Credit when married filing separately.

An example given in the instructions for Form 5049 helps describe how to properly fill out this form if you are filing separate, but have lived together for the entire year. I have attached a snippet of the example below.
