Box 14 and supplemental statement

I need help with category and employer state ID #....it is blank on my w2

    Typically if there is not a category that matches your W2 box 14, you select "Other (not classified)".  

     

    As for the employer state ID since it is blank and assuming there are amounts in boxes 16 and/or 17, you can use the federal ID number shown on the W2 without any blanks/dashes. 

     

    If there are no amounts shown in the other state boxes just delete the state line.
