Cobra Subsidy in Massachusetts State Filing

Greetings. 

I was laid off in mid/late August of 2021 and my former employer paid 100% medical for 2 months for both my husband and I and then I purchased COBRA from then on.  Massachusetts state form is requesting an amount of "Cobra Subsidies" if received between April and September 2021.  Is this "COBRA Subsidy" the value my former employer paid from August 21st through the end of September?  Is that what the MA State form is looking for?  and if so, it is only through end of September of all monies my former employer paid for our medical for those two months?

Sorry - this is a first time for me and I am unclear and want to make sure I do this correctly.

    The medical insurance paid by your employer does not appear to be COBRA premium assistance since you purchased COBRA after your employer paid insurance expired.  Additionally you needed to apply for the premium assistance by 31 July 2021.  Therefore the premiums paid by your employer are not COBRA subsidies as defined under the American Rescue Plan.
