Do I need to file with WV?

My wife and I have fully lived in North Carolina for over 15 years. She received a settlement check (from an old job) for roughly $350 last year and we just received a W-2 related to it. The W-2 has her maiden name and old West Virginia address. 

Given this fact, do we need to file a separate WV tax return for the roughly $350?

    No, you would not need to file a return for WV since her income is less than $2,000.  WV has a $2,000 exemption so because her income is only $350, you do not need to file a WV return.
