Filing IL State taxes for my deceased mother.

The tax.illinois.gov website says:

When you are filing a return on behalf of a single deceased taxpayer,

  • print the name of the taxpayer on the appropriate line.
  • write “deceased” and the date of death above the decedent’s name.
  • write “in care of,” and the representative’s name and address.

A personal representative, such as an executor or administrator must sign and date the return.

 

My questions are:

     Where is the "appropriate line" (bullet 1)

     Where do I write "in care of" and my address (bullet 3)

 

Turbotax added "Deceased and the date" on page 1 of my 1040 return.  Is that what is being referred to for bullet 1?   What about bullet 3?  Turbotax didn't do anything with that.

    Bullet 1 and 2 refer to the heading of Illinois form IL-1040.

     

     

    "In care of" refers to signing the tax return in the name of the deceased on the bottom of page two.

     

    Make sure that the correct address is reported on IL-1310.

    Thank you!  Don't know why Turbotax didn't ask me to fill out the IL1310 form to begin with.
