Forgot to mail the first page of my state(OH) tax return.

As the title says, I came home from the post office only to see the first page of my state tax return sitting on my desk with some other miscellaneous papers. I don't know how it got separated. It's the first page with my Name,SSN,Address,zip code,federal adjusted gross income,adjusted gross income, all that stuff. Are things going to be ok without it? Most of that info is available on other pages of the return. Will they be able to send me my tax return? If not, how do I go about sending an amended return?

    You don't need to file an amended return, because you do not have any changes to make.  

     

    Print out the rest of the pages to add to the first page to form a complete return. ‌Write a brief statement explaining that when you filed your tax return originally, the first page was omitted inadvertently, and you are now submitting the complete and accurate return.
