TurboTax is looking for a number here.  Their guidance says:

Amounts deducted for health insurance and the welfare benefit fund surcharge as reported on your:
- New York State Form IT-2102 (Copy 1), or
- Federal Form W-2 (Copy 2)

This applies to members of the:
- New York City Employees' Retirement System
- New York City Board of Education Retirement System

Just where on my W-2 can I find this?

Health insurance that is deducted on your check is usually reported in Box 12 under a DD code. The Welfare surcharge insurance/benefits surcharge on your W2 is the IRC125. It is located in Box 14. If this is blank, there should be a number there or just leave it out entirely.  i think this is what Turbo tax is looking for. I have seen where entries were made in box 14 referencing  IRC125 but left blank.

    Hi @DaveF1006

I have Box 14 with two items: SDI and PFL. Can I enter these into "Enter Insurance and Benefits Surcharge: Enter any health insurance and welfare benefit fund surcharge that your employer deducted in 2022"?

I also have Box 12a: DD 6657.60. Is this amount needs to be entered in above mentioned field?

So would that mean you’re only entering DD if there is no IRC?

I see this is a very old post, but I have the same question. I put in my amount under 12b DD and my state taxes sky rocketed. Not sure how this makes sense since it's not taxable?

The health benefits in box 12 DD are not taxable at the federal level. Some states (e.g., New Jersey) might require additional information in the state return about your health benefits. When you do your state return, answer all of the interview questions.

In addition, check to see if you designated any of your W-2 as Paid Family Leave. If so, there may be additional questions to answer in your state return (e.g., California).

This help article confirms that the amount doesn't affect your Federal return.
