Question

How do I attached form 804 to Wisconsin State return. My mother is deceased and it is my authorization

It depends. If you mail the tax return you can include the Form 804 with your mother's tax return. This form is not available in TurboTax and is not included as a pdf attachment.

TurboTax will provide the mailing address and it is placed here for your convenience:

  • Forms filed on paper may be mailed to: Wisconsin Department of Revenue, PO Box 8920, Madison, WI 53708-8920
