How do I file1099-R tax return for a death benefit?

You need to enter that in the section for Retirement Distributions. The code "4" in Box 7 indicates that it is a death benefit.

Choose the Federal section from the menu on the left, in black.

Choose Income & Expenses from the menu at the top.

Scroll down to IRA, 401(k) Pension Plan Withdrawals (1099-R)

Follow the on-screen prompts.
