How do I know if I had any Nj income tax withheld ?

New Jersey tax that was withheld by your employer would appear on your W-2.

 

Box 15 on the W-2 should have "NJ" followed by the employer's ID number. Box 16 should be the amount of income for the state, and box 17 should be the amount of state income tax withheld.

 

A similar thing happens on the 1099-R - your state tax withheld appears in box 12.
