Are they looking for the date I moved to Michigan?

Based on the context of the instructions at RESIDENCY STATUS, in the 2021 Part-Year form and Instructions linked as 2021 Part Year at Income Tax Forms, the dates requested would be those you resided in Grand Rapids, as opposed to the state of Michigan.

The form is requesting dates of residency, To and From, in a mm/dd/yyyy format.
