I do not have a Certificate of Rent Paid worksheet, and I cannot move forward with the application without it. I do not have a physical copy of the M1M form.

In order to move forward, you can request a Certificate of Rent Paid (CRP) and obtain a physical copy of the M1M form.

Certificate of Rent Paid (CRP):

• Contact Your Landlord: Reach out to your landlord or property manager and request a new copy of the CRP. They are required to provide this document to you by January 31 each year

M1M Form:

• Request by Mail: You can request a physical copy of the M1M form by calling the Minnesota Department of Revenue. See contact information here.
