Question

I moved to AZ in August for School. School didn't work out. Moved back to WA in December. Do I put the date I moved back as establishing residency?

If you were living away from your parents' home ---going away to school was considered to be a "temporary absence" from your home state, so you do not need to re-establish residency there.
