Solved

I received a letter from the Ohio Department of Taxation. It says I didn’t pay my 2014 school district tax. Can you view my account and see if it e-filed?

In the PDF there is a section under the Ohio tax return that reads "SD 40P School District Income Tax Payment Voucher". It also says "If you are sending this voucher and paper check or money order (payable to School
District Income Tax) with or separately from your school district income tax return, mail
to: School District Income Tax, P.O. Box 182389, Columbus, OH xxxxx-xxxx. Write the
last four digits of the taxpayer’s Social Security number on the check or money order."

Was I suppose to print this and mail a check? I didnt have to do that last year?

Thanks in advance for any info you can provide.

Steve

<Update> Thanks for the info. I didn't realize. I will get them a check. Thanks again.

Yes, you were to print the SD40P and mail a check.

You should verify that your were required to file a school district return. To find out if you live in a school district  with an income tax enter your address at:
