Solved

I think that there is an error in my tax. Lake County, IN imposes a 0.25% tax rate on non-residents. My Indiana tax filings through TurboTax for 2014, 2015, 2016 error.

0.5% is actually correct.  Please notice this bulletin:  Lake County LOIT FAQs - IN.gov. (Click on link, it is a PDF file)  The tax was instituted in 2013, and since it was late in the year, they prorated the amount to 0.125% that year.  But since 2014 the amount was 0.5%

Do you live out-of-state?  Which State?

So you should have filed an Indiana non-resident return, and it should have stated that you lived out-of-state but worked in Lake County.  Is that what you inputted for those years?
