I work in Oregon and have a 401k. I took $ out in 2023.Does this get applied as income on the Oregon State form?

Yes. Some states exclude 401(k) withdrawals from income, and some don't. Oregon doesn't. So, your 401(k) withdrawal will not appear as a federal subtraction on the OR-40 return.
