Kentucky Exempt-Interest Dividends

I have exempt interest dividends and live in Kentucky. The fund company doesn’t breakdown ALL the states, etc. where the dividends come from, only the top 10. How do I proceed?

    If your fund company doesn't break down all the states for your exempt-interest dividends, then you can list the states you do have listed and then choose Multiple States for the rest. You will get to this screen after entering your 1099-DIV, Box 12. 

     

    Example:
