LTC E-Filer Attachment Form for LTC Premiums paid in Colorado

After E-filing for Federal and Colorado Turbotax says I must print, sign and MAIL in an E-filer Attachment form, but there is no explanation about why this message came AFTER I submitted the E-filing. Why do I need to print and sign this and where do I mail it? Also, can this single page form be mailed in or do I have to print a hard copy of my Colorado return to go with it?  If so, what was the advantage of paying for E-filing for the state?

    The required documents may also be submitted electronically through the E-Filer Attachment function of Revenue Online at E-filer Attachments. You do not need to log in or create an account to upload e-filer attachments via Revenue Online for your individual income tax return.
