I pay less than $500 per year in Maryland taxes. Nothing is withheld. Why would I have to pay estimated taxes

You do not have to. TurboTax suggests estimates, but it is always your choice to pay them. $500 is the threshold for required estimated payments for Maryland, so if you are comfortable making the payment at once at tax filing time, you can disregard the suggested payments (Estimated Payments).
