Massachusetss Schedule HC is asking me to check Line 10. Where is it?

I worked for two companies both offered insurance but I did not take insurance from first company because they wanted $700 a month which I could not afford. Left that job and took another which offered insurance and I took it but had a 90 day wait period. So MASS is wanting a penalty for not having insurance. Filled out worksheet and after putting in all info, it was deemed I could not afford and to check No oval on LINE 10 of Schedule HC but cannot find. Do I appeal? Or does Turbo have the wrong form?
