We bought medical and dental insurance from the company my husband work. We, as an employee, paid $4180.02 a year out of our paycheck and the employer paid $17,315 a year. What amount should we put on L31 -Medical Insurance / HMO premiums you paid for you and your family of the Form MI-1040CR? Should we put:

 

1. $0;

2. $4,180.02; or

3. $21,495

NB: In the instruction, it says “Do not include any premiums paid by an employer with pre-tax payroll contributions.”

It depends if the $4180.02 were paid with pre-tax dollars, which most premiums paid by employers are. If the $4180.02 was included in your wages that are reported in Box 1 of your W2, you would report $4180.02. If that amount was excluded from your gross pay reported in Box 1, then the amount is $0 because it was paid with pre-tax dollars.
